Membership Documents

So you're thinking about becoming a member? Or maybe you're asking yourself why should I become a member what are the benefits to being part of this club..... Well good question! The list is a bit long so click this BENEFITS BUTTON to see our list of Growing benefits. So have you decided that you would like to become a member of the West Florida Dive Club? Well here are all the documents that you will need to become a member. You can bring display and print those documents, fill them out and bring them to our next meeting along or we'll have a copy at the meeting for you to fill out. The documents are in MS Word and to display the document for printing click on the highlighted document. The Documents need are:

1.) The Membership application

2.) Liability Release Form

In addition you must have some form of Dive Insurance such as DAN, Dive Assure, Tri-Care, etc.

And we will need a copy of your C-Card and your Dive Insurance for our records.
